The SBAR Handover Prompt Card has been designed by serving HCPC registered Paramedics with input from PPs (Paramedic Practitioners) and senior nursing staff who all have experience in patient handover and clinician to clinician patient discussions. The prompt card is there to assist medics of all grades to assist with a patient handover that is not time critical or when discussing a particular patient with another medical professional.
