SAMPLE history is a mnemonic acronym to remember key questions for a person’s medical assessment. The SAMPLE history is sometimes used in conjunction with vital signs and OPQRST. The questions are most commonly used in the field of emergency medicine by first responders, Emergency ambulance crews and even hospital staff.

We recommend using this aide-memoire with a permanent fine-tip marker to prevent smudging. To clean the card after use, we suggest using both a clinell wipe and a small part of alcohol gel.
